REGISTRATION

To save time, you may wish to complete the registration form before coming to the office.  To do so, click on the link below, print the registration form and fill out the form as completely as possible.   Be sure to also bring your insurance card and any referral forms.  If you have also down loaded the medical history form from this web site, be sure to complete it and bring it with you.  If you have had any x-rays, CT scans or MRI's, be sure to obtain them from where they were done and bring them with you.  It is important that we see the films and not just the report.
